On or about and at times prior to September 10, 2025, at the establishment located at N4380 County Road E, Plymouth, Wisconsin; the employer did not ensure that flexible cords were not used as a substitute for the fixed wiring of a structure, exposing employees to electrical hazards in the following instances:  a) A flexible cord powering the Box Erector (Model: GPK-40E, SN: 14-2204015-1) had been used for an extended period of time. b) A flexible cord powering the Box Taper  had been used for an extended period of time and was affixed to stationary materials using zip ties.
