29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface that have side rails that extend at least 3 feet (0.9 m) above the upper landing surface. Pan Building - Cable turning employees using portable hook on ladders to enter and exit the pans were not protected from a fall of up to 7 feet where the portable hook on ladders did not have side rails extending three feet above the landing surface.

29 CFR 1910.179(n)(3)(vi): The employer did not require that the operator of crane(s) avoid carrying loads over people: Pan Building- Employees are not protected from struck by hazards where the 110-ton gantry crane moves pans over the heads of employees working in the building. On or about September 6, 2025, an employee was struck by the superstructure of the 110-ton gantry crane when it entered the Run Over Department without stopping and ensuring that no one was in the area while it moved a pan to the North door. On or about September 11, 2025, an employee moved a pan through the Run Over Department with the 110-ton gantry crane over the heads of employees while going to the North door during the onsite OSHA inspection.
