29 CFR 1910.178(l)(4)(ii)(B): Refresher training and evaluation was not provided to the operator when the operator was involved in an accident or near-miss incident. (a) On or about August 13, 2025, two forklift operators were involved in a forklift accident. The employer did not ensure the forklift operators received refresher training on safe forklift operations and an evaluation prior to allowing the forklift operators to return to forklift operation.
