29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Westlake Tool & Die on or about September 9, 2025, at the location of 1280 Moore Road Avon, Ohio 44011: The employer failed to adequately guard Line 215 coil cradle, exposing employees tasked with operating and monitoring the equipment to a sheer point hazard.
