29 CFR 1926.28(a):The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. a) On or about September 12, 2025, employer failed to provide the appropriate personal protective equipment, such as gloves, safety steel toe shoe, and hard hat to the employee engaged in removing heavy debris work underneath an aerial lift.
