29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about September 11, 2025, employees engaged in excavation activities at the Hawkins Construction Company jobsite located at the intersection of 40th and Farnam Streets, in Omaha, NE 68131, were exposed to struck-by and caught between hazards related to potential cave-in working conditions in that the employer had employees working in an unprotected trench that was 5-feet 6 and ?-inches deep from the top bench to the base of the trench where employees were walking/working.
