29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about September 11, 2025, employees in the process of completing roofing activities at a two-story 6-in-12 pitched roof located at 2530 Bearberry Ln, Johnstown, CO, were exposed to fall hazards of approximately 20 feet. The employer failed to ensure employees were utilizing conventional fall protection (e.g.: harness and lanyard, lifeline, anchor points) and/or other devices.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about September 11, 2025, employees in the process of completing roofing activities at a two-story 6-in-12 pitched roof located at 2530 Bearberry Ln, Johnstown, CO, were exposed to fall hazards of approximately 20 feet. The employer failed to provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of framing work.
