29 CFR  1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  a) 554 Lincoln Ave., Winnetka, Illinois- On September 12, 2025, the employer did not ensure that all persons who operated vehicle-mounted elevating and rotating work platforms, such as the Ford Super Duty Versalift Telescopic Aerial Lift, donned a harness or body belt and lanyard attached to the boom or basket while removing holiday lighting.  b) 820 Elm St., Winnetka, Illinois- On September 12, 2025, the employer did not ensure that all persons who operated vehicle-mounted elevating and rotating work platforms, such as the Ford Super Duty Versalift Telescopic Aerial Lift, donned a harness or body belt and lanyard attached to the boom or basket while removing holiday lighting.  No abatement certification or documentation is required for this item.

29 CFR  1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  a) 820 Elm Street, Winnetka- On September 12, 2025, the employer did not require the use of head protection, such as hard hats, for employees that were exposed to falling tree branches and debris during tree decorating.    No abatement certification or documentation is required for this item.

29 CFR  1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:  a) 565 Chestnut Street, Winnetka- On September 12, 2025, the employer did not ensure that ladders used by employees, such as the blue Werner Fiberglass A-Frame Ladder, were placed on level surfaces.  The ladder straddling the street and a sidewalk while being accessed by an employee installing holiday lights.   No abatement certification or documentation is required for this item.
