1910.145(f)(5) Danger tags were not used where an immediate hazard presented a threat of death or serious injury to employees: a) Washing Department - One or more methods of machine guarding was not provided to employees in the machine area from hazards created by rotating parts of a washing machine. Employees are exposed to amputation hazards as there was not a functioning means of safeguarding on washing machine #2. An employee performing job tasks of loading and unloading washer machine #2 suffered an arm amputation due to inadequate guarding of a washer machine on or about 9/15/2025.
