29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections. a) Production floor: On or about 9/13/2025, the employer did not ensure that the RUSH Machinery, Truing and Dressing Machine, MODEL NO. FC-250W, SERIAL NO. RF-9480, was effectively guarded. The safety guard was affixed to the machine by a single thumbscrew that was slid, not threaded, into one of the two mounting holes, enabling potential movement of the guard and a possible exposure to the rotating wheel.
