29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about September 16th, 2025 Location: 87 Maple St, Croton-On-Hudson, NY 10520  a) Employees were exposed to approximately 20 ft fall while accessing a roof with an extension ladder that did not extend at least 3 ft above the top landing. The ladder extended only approximately 1 foot above the landing level.
