Wichita, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 10 feet from the eave of the residence to the ground below while accessing and egressing the roof. The portable extension ladder did not extend at least 3 feet above the upper landing surface or was not secured to the residence.
