29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about September 16, 2025, the employer failed to develop, document, and utilize machine specific procedures for clearing of jams on machines, including but not limited to, the Crimpers. An employee sustained a finger amputation when clearing a jam on the VEVOR Pneumatic Crimping Machine S/N 241226-4310-1-0001 Crimper. The machine was not locked and tagged out and there was no procedure in place for this machine.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On and before September 9 2025, the employer failed to ensure machine guarding was adequately provided to protect operators from machine hazards created by the point of operation of the VEVOR Pneumatic Crimping Machine S/N 241226-4310-1-0001. An employee sustained a finger amputation.
