29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: a) Deli Dish Room, September 16, 2025, the employer did not ensure the unused outlet cover plate was installed after it went missing from the wall. Employees were exposed to electrical hazards when putting on or taking items off the shelf in front of the uncovered outlet.
