On or about September 17, 2025, in an excavation located in the parking lot near 815 W. Danforth Road, an employee entered an excavation with a depth greater than 4 feet without a safe egress while performing utility locating operations.

Danforth Road, an employee worked in an excavation while a backhoe was within 2 feet of the edge of the excavation, while performing utility locating operations.
