29 CFR  1926.502(d): The employer used body belts as part of a personal fall arrest system:  (a) Residential roof, various locations; On or about September 16, 2025, roofing employees were permitted to use waist level attachment points at the front/side of body belts (climber style harnesses) as part of their personal fall arrest systems during roofing activities at heights ranging between approximately 9-feet, 4-inches to 20-feet, 6-inches above ground level.  Note: As of January 1, 1998, subpart M of this part (Sec. 1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a positioning device system for vertical surfaces, such as working on a wall, is regulated under Sec. 1926.502(e).

The upper side rails of the ladder did not extend at least 3-feet above the landing surface.
