On or about September 15, 2025,  in Theory and Computer Sciences (TCS) Building #240 at Argonne National Laboratory, the employer did not ensure that locks and/or tags were applied to the electrical feed(s) for Transformer 15A prior to allowing employees to begin work.  Employees were exposed to electrical shock hazards while replacing the bushings of Transformer 15A.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees were testing the bushings of Transformer 15A using a multi-meter, rated for a maximum of 1,000 volts, on an energized component operating at a nominal voltage of 13,200 volts.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
