29 CFR 1910.333(b)(2): "Lockout and Tagging." While any employee is exposed to contact with parts of fixed electric equipment or circuits which have been deenergized, the circuits energizing the parts shall be locked out or tagged or both in accordance with the requirements of this paragraph. On or about September 15, 2025, in Theory and Computer Sciences (TCS) Building #240 at Argonne National Laboratory, the employer did not ensure that locks and/or tags were applied to the electrical feed(s) for Transformer 15A prior to allowing employees to begin work. Employees were exposed to electrical shock hazards while replacing the bushings of Transformer 15A. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.334(c)(3): Rating of equipment. Test instruments and equipment and their accessories shall be rated for the circuits and equipment to which they will be connected and shall be designed for the environment in which they will be used. On or about September 15, 2025, in Theory and Computer Sciences (TCS) Building #240 at Argonne National Laboratory, the employer did not ensure electrical circuits were tested with rated equipment. Employees were testing the bushings of Transformer 15A using a multi-meter, rated for a maximum of 1,000 volts, on an energized component operating at a nominal voltage of 13,200 volts. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about September 15, 2025, in Theory and Computer Sciences (TCS) Building #240 at Argonne National Laboratory, the employer did not ensure electrical protective equipment was used at Transformer 15A. An employee received an electrical shock while checking voltage inside the cabinet without electrical protective equipment. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
