Workshop - On or about September 17, 2025, and days prior to, the employer failed to ensure that employee(s) who operate powered industrial trucks, including the Komatsu Forklift, Model FG45TU-10, Serial No. 138149 and Toyota Forklift, Model EFGCU15, Serial No. 14858, were not properly trained, evaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.
