29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements. a) Concrete Recycle Yard Supply Trailer in Riverview, FL- On or about July 17, 2025, employees were exposed to fall hazards while using a set of portable metal stairs with five risers that lacked the required stair rail system on both sides. Employees used these stairs to access equipment and supplies inside a storage container, placing them at risk of falling due to inadequate fall protection.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Concrete recycle area - Riverview, FL: On or about July 17, 2025, employees were exposed to injury hazards while performing maintenance and/or servicing tasks on machinery, including but not limited to the rock crusher. The employer had not established or implemented written lockout/tagout procedures to ensure that all sources of hazardous energy were isolated and controlled prior to employees performing this work.
