29 CFR 1910.134(a)(2): A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the employee: a)Top coating room - On or about September 10, 2025 and at times prior, the employer exposed employees to respiratory hazards in that, the employer failed to ensure the respiratory protections were provided to employee using Methyl Ethyl Ketone (MEK) while flushing the paint line at the end of the shift.

29 CFR 1910.1000(a): The employer shall ensure that no employee is exposed to any substance at a level in excess of the exposure limit given for that substance in Table Z-1 of this standard. a) Welding area, on September 24, 2025, the employer exposed employees to health hazards associated with Zinc Oxide fume in that, the employer failed to ensure that employees were not exposed to Zinc Oxide fume at a concentration exceeding the OSHA PEL of 5 mg/m� when air sampling conducted for welding employees indicated exposure level of 8.16 mg/m3.
