OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by, caught by, and crushing hazards due to unstable and damaged shelving systems:  a.) Tractor Supply Co., 425 Beaver Drive Dubois, PA 15801 on or about September 3, 2025, and times prior thereto: Two employees were working near the cat food aisle when the Lozier L-Line Basic Gondola shelving unit collapsed onto employees. The shelving unit was missing the kickplates and improperly loaded with heavier items that include, but is not limited to, cat food on the North side and lighter items on the South side which include, but is not limited to, cat toys and bedding.   b.) Tractor Supply Co., 425 Beaver Drive Dubois, PA 15801 on or about September 18, 2025, and times prior thereto: Cat food aisle #4 shows signs of damage to the bottom portion of the shelving system. The damage observed includes, but is not limited to, bent baseplates, levelers, and shelves.   c.) Tractor Supply Co., 425 Beaver Drive Dubois, PA 15801 on or about September 18, 2025, and times prior thereto: Kickplates which provide structural integrity to the shelving system were removed throughout the store. Multiple aisles were observed with missing kickplates at the bottom of the shelving systems.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
