On or about September 18, 2025, on the roof of a residential roofing project, employees were not in use of fall protection while performing roofing work activities from a roof that was more than six (6) feet from the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about September 18, 2025, on the roof of a residential roofing project, employees performing roofing work activities accessed the roof using ladders with siderails that did not extend at least three (3) feet above the landing.
