29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) On or about September 18, 2025, the employer had not developed and utilized machine specific lockout-tagout procedures for the middle flag cutting line in the production area. Employees were exposed to amputation hazards during the unjamming of the flag cutting shear on the middle flag cutting line in the production area. (b) On or about September 18, 2025, the employer had not developed and utilized machine specific lockout-tagout procedures for the easternmost flag cutting line. Employees were exposed to amputation hazards during the servicing and maintenance of the flag cutting shear on the easternmost flag cutting line  in the production area.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about September 18, 2025, the employer did not ensure that employees operating the Edward Segal Inc. flag grommet machine, Model No: 2KGW, S/N:5790 were protected from amputation hazards during the application of grommets to flags.  (b) On or about September 18, 2025, the employer did not ensure that employees operating the Edward Segal Inc. flag grommet machine, Model No: 2KGW-E, S/N:6059 were protected from amputation hazards during the application of grommets to flags.  (c) On or about September 18, 2025, the employer did not ensure that employees receiving product from the shear for the middle line of the flag cutting machines in the production area, with no locatable Manufacturer or Model Number identifiers, were protected from amputation hazards during the cutting of flags from rolled material to individual flags. Openings were present on the discharge side of the shear which allowed access to the shear while in operation.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
