29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Commercial building rooftop, various locations; On or about September 18, 2025, employees were not required to wear eye protection to protect against flying particles while operating pneumatic nail guns to affix asphalt shingles using 1-1/4-inch coil roofing nails.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) Commercial building rooftop, various locations; On or about September 18, 2025, and dates prior, the employer did not provide training on applicable topics, including but not limited to, fall hazard recognition and mitigation procedures, regarding employees who were not protected from falls while engaged in roofing replacement activities from a steep-slope roof at approximate heights of up to 10.5 feet above ground level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Commercial building rooftop, west side; On or about September 18, 2025, and dates prior, a 24-foot Keller brand fiberglass extension ladder was leaned against the west-facing eave and was used for rooftop access/egress where the landing surface was approximately 10.5 feet above ground-level.  The upper side rails of the ladder did not extend at least 3-feet above the landing surface.
