29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   On or about September 19, 2025, employees performing roofing operations were exposed to eye injuries and fall and overhead hazards and the employer did not ensure a competent person inspected the jobsite for unsafe activities and take necessary steps to eliminate and control the hazards:  a. Employees on the roof were exposed to fall hazards greater than 10 feet without fall protection.  b. Employees working on the ground beneath employees on the roof performing roofing activities were exposed to overhead hazards without head protection.  c. Employees on the roof were were exposed to eye injuries while using pneumatic nail guns without eye protection.   d. Employees using portable ladders were exposed to fall hazards because the ladders did not extend three feet over the roof edge.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   On or about September 19, 2025, employees using portable ladders to access the roof were exposed to fall hazards and the employer did not ensure the ladders' side rails extended at least three feet above the roof's eave:  a. The multi-position ladder used to access the South-facing side of 3941 Pintail Drive extended approximately 1.5 feet above the roof's edge.  b. The multi-position ladder used to access the North-facing side of 3941 Pintail Drive extended approximately 1 foot above the roof's edge.  c. The extension ladder used to access the North-facing side of 3930 Wood Duck Drive extended approximately 2 feet above the roof's edge.
