29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about September 19, 2025, on the roof of a residential roofing, employees of a subcontractor were not in use of fall protection while performing roofing work activities in excess of six(6) feet from the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about September 19, 2025, at the front of a residential structure, employees of a subcontractor accessed the roof to perform roofing activities by using a ladder that did not extend three (3) feet above the eave.
