OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to struck-by hazards:  On or about September 18, 2025, and at times prior thereto, at the Alta Mesa drilling site, employees were exposed to struck-by hazards from the sudden release of high-pressure fluid from a mud pump while conducting reaming operations.

29 CFR 1910.145(c)(3):  Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s):    On or about September 18, 2025, and at times prior thereto, at the Alta Mesa drill site, the employer did not ensure that hopper and downhole valves on the duplex mud pump were marked or identified with accident prevention signs or tags to distinguish them from one another, which could result in improper valve sequencing and an uncontrolled release of high-pressure fluid.

29 CFR 1910.219(e)(1)(i):  Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o):  On or about  September 18, 2025, at the Alta Mesa drill site, employees utilized a mud double slush pump with an exposed belt while engaged in pipe reaming activities.

29 CFR 1910.303(b)(1)(vi):  Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees.     On or about September 18, 2025, at the Alta Mesa site, at the mud double slush pump operating station, electrical wiring and connectors were exposed beneath the control panel top surface and were not enclosed or otherwise protected, exposing employees operating or servicing the pump to electrical shock hazards.
