29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  East and north roof edges; On or about September 22, 2025, employees engaged in residential roofing activities were exposed to falls to lower levels ranging from approximately 8 feet 9 inches to 19 feet 3 inches without being protected by one or more methods of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) East roof edge; On or about September 22, 2025, while using a Safe Reach 300-lb capacity multi-position ladder, set up as a stepladder, employees accessed the east-side roof without configurating the ladder to have the side rails extended 3 feet or more above the roof surface.
