29 CFR 1910.332(b)(1):Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: On or about June 27th, 2025, employees engaged in locating operations located at 2525 North 117th Ave, Omaha, Nebraska 68117, were exposed to electrical shock in that employees were accessing the panels of street lights which contained an energized 120V and they were attaching their locating leads to a circuit contained within the light pole in order to locate and mark underground energized utilities. The employer did not properly train employees on how and when to use electrically rated gloves (PPE) when there is potential for employees for employees to come in contact with energized electrical circuits.

29 CFR 1910.335(a)(1)(i):Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: On or about June 27th, 2025, employees engaged in locating operations located at 2525 North 117th Ave, Omaha, Nebraska 68117, were exposed to electrical shock in that employees were accessing the panels of street lights which contained an energized 120V and they were attaching their locating leads to a circuit contained within the light pole in order to locate and mark underground energized utilities. The employer does not require employee to wear electrically rated gloves (PPE) when performing these operations.
