29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about September 23, 2025, employees conducting roofing activities on a residential structure to include tear-off,  and roof installation on a steep pitch roof were not protected from falling to the lower level by a personal fall arrest system, or other means of fall protection. This exposed employees to a fall hazard of in-excess of 22-feet onto concrete, grass, and building materials.  C & C Exteriors LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1656877, citation number 1, item number 1, and was affirmed as a final order on July 26, 2023 with respect to a workplace located at N6145 Jefferson Rd Johnson Creek, WI 53038.  C & C Exteriors LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1589952, citation number 1, item number 2, and was affirmed as a final order on September 27, 2022, with respect to a workplace located at 3043 N Oakland Ave, Milwaukee, WI 53211.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  (a) On or  about September 23, 2025, the portable extension ladder being used by  employees to access the  roof on the West side did not extend at least three feet above the upper point of contact with the residential structure, nor were ladders secured in place. This exposed employees to a fall hazard of 22-feet, 6-inches onto grass,  and concrete.  (b) On or  about September 23, 2025, the portable extension ladder being used by  employees to access the  roof on the South  side did not extend at least three feet above the upper point of contact with the residential structure, nor were ladders secured in place. This exposed employees to a fall hazard of  22-feet, 6-inches onto grass,  and concrete.  (c) On or  about September 23, 2025, the portable extension ladder being used by  employees to access the  roof on the East side did not extend at least three feet above the upper point of contact with the residential structure, nor were ladders secured in place. This exposed employees to a fall hazard of 12 feet onto grass, and concrete.  C & C Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1656877, citation number 1, item number 2, and was affirmed as a final order on July 26, 2023 with respect to a workplace located at N6145 Jefferson Rd, Johnson Creek, WI 53038.
