(c) On or  about September 23, 2025, the portable extension ladder being used by  employees to access the  roof on the East side did not extend at least three feet above the upper point of contact with the residential structure, nor were ladders secured in place. This exposed employees to a fall hazard of 12 feet onto grass, and concrete.
