29 CFR 1910.1200(e)(1):Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: On or about September 23, 2025, the employer did not develop, implement and maintain a written Hazard Communications Program, exposing employees to hazardous chemicals such as silica. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system: On or about September 23, 2025, employees were not protected from a fall while working from a ladder jack scaffold, exposing employees to a fall of 17 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
