On or about 19 September, 2025, the employer did not ensure that eye protective equipment was used when employees were exposed to eye injury while using pneumatic nail guns.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Location : 164 Main St. Kingston MA.   On or about September 19, 2025, employees were exposed to falls from heights up to 22 feet to the ground while accessing an upper landing surface where an extension ladder was used to access the roof where the ladder side rails did not extend at least 3 feet above the edge of the roof.
