29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about September 23, 2025, employees conducting roofing activities on a residential structure to include tear-off, and shingle installation on a steep pitch roof were not protected from falling to the lower level by a personal fall arrest system, or other means of fall protection. This exposed employees to fall hazards of 12 feet 7-inches at the lower eave to in-excess of 30-feet at the gabled ridge end onto concrete. (b) On or about September 23, 2025, employees conducting roofing activities on a residential detached garage structure to include tear-off, and shingle installation were not protected from falling to the lower level by a personal fall arrest system, or other means of fall protection. This exposed employees to a fall hazard of 8 feet 10-inches onto concrete.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, or were not secured: (a) On or about September 23, 2025, the portable telescoping ladder being used to access the roof of the main house on the East side of the structure did not extend at least three feet above the upper landing area and was not secured. This exposed employees accessing and egressing to and from the roof, to a fall hazard of approximately 12-feet 10-inches onto grass, building materials, and concrete. (b) On or about September 23, 2025, the portable extension ladder being used to access the roof of the detached garage on the West side of the structure did not extend at least three feet above the upper point of contact and was not secured. This exposed employees accessing and egressing to and from the roof, to a fall hazard of approximately 8-feet 10-inches onto building materials, and concrete.
