29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: 29 CFR 1926.454(a)(3): The proper use of the scaffold, and the proper handling of materials on the scaffold; a. Chuy New Carpentry LLC at a job site located at 4530 Generosity Way, Hilliard, OH 43026: On or about September 23, 2025, the employer failed to ensure that each employee exposed to a fall hazards while working on a Genie GS 932m scissor lift (scaffold) was trained on the correct use of the lift, hazards of falling and the procedures to minimize the hazards in the work area. As a result, employees were exposed to fall hazards of over 14 feet while standing on the equipment railing performing commercial construction.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a. Chuy New Carpentry LLC at a job site located at 4530 Generosity Way, Hilliard, OH 43026: On or about September 23, 2025 , the employer failed to ensure that each employee exposed to a fall hazard was trained on the hazards of falling and the procedures to minimize the hazards in the work area. As a result, employees performing framing work on elevated surfaces were exposed to fall hazards of over 14 feet.
