29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) Workplace, Production Area -The employees were performing adjustments to the Elmar Filler Machine without having received adequate instruction on the safe application, usage, and removal of the electrical and mechanical energy controls; on or about 9/19/25. Note: the employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The employer did not report the amputation to OSHA within the following 24 hours; on or about 09/20/25 . Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
