29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Restaurant Brands International Inc., DBA Carrols Corporation, Burger King, 955 West Genesee Street, Syracuse, New York 13204, on or about September 15, 2025; At approximately 10:00 am, an employee slipped on the floor and was admitted to a hospital with a broken leg. OSHA was not notified of the hospitalization until, September 24, 2025 at 1:01 pm.
