LOCATION: The front lawn between the building and the street at 431 Pond St. On or about September 19th, 2025, inspections of excavations, the adjacent areas, and protective systems were not made by a competent person.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). LOCATION: The front lawn between the building and the street at 431 Pond St. On or about September 19th, 2025, workers were exposed to cave-in and crushing hazards when the employees in an excavation in excess of 9 feet were not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).
