29 CFR  1910.36(d)(1):  Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  Ammonia Machine Room:  On, or about, September 23, 2025, the exterior exit door was secured closed with a deadbolt.    Abatement Note:  International Institute of Ammonia Refrigeration (IIAR) Standard 2 - 2021 states that doors that are part of the means of egress from Machinery Rooms shall be equipped with panic hardware.

29 CFR  1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Machine Room:  On, or about, September 23, 2025, adequate eye and body flushing equipment was not provided where an anhydrous ammonia refrigeration system was in use.  A body wash unit was not provided inside of the Machine Room and an eye/body wash unit capable of providing at least fifteen minutes of continuous flow was not provided outside of the Machine Room.  ETR Operating, LLC dba Enfield Twin Rinks was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 1799269, Citation Number 1, Item Number 2 and was affirmed as a Final Order on June 17, 2025, with respect to a workplace located at Enfield, Connecticut.

The ladder had been set up directly on the ice.
