The top rail and mid rail at one of the platforms used by employees were not installed at the required heights and this condition lasted for several months.

Location: 55 John Street, New York, NY.  On or about 9/24/2025,  a)  The top rail of tubular supported scaffold was installed at approximately 35 inches above the platform surface. An employee fell from the scaffold platform while performing masonry work.
