29 CFR  1910.29(b)(13)(i): The guardrail system opening used around holes that serve as points of access (such as ladderways),  did not have a self-closing gate that slides or swings away from the hole, and equipped with a top rail and mid rail or equivalent intermediate member that met the requirements in paragraph (b) of this section.  (a) Third floor of the coating area - On or about September 24, 2025, and at times prior to, the employer exposed employees to a fall hazard, in that the guardrail system was not equipped with a self-closing gate for the ladderway on the elevated catwalk for the recirculation conveyor with a 9' fall to floor distance.  (b) First floor of the coating area - On or about September 24, 2025, and at times prior to, the employer exposed employees to fa fall hazard, in that the guardrail system was not equipped with a self-closing gate for the ladderway on the elevated catwalk for the Line 2 cooler with a 53" fall to floor distance.

29 CFR 1910.23(c)(2): The employer did not ensure that each stepladder or combination ladder used in a stepladder mode is equipped with a metal spreader or locking device that securely holds the front and back sections in an open position while the ladder is in use.  a) On the first floor of the coating area - On or about September 24, 2025, the employer exposed employees to a fall hazard, in that employees used a stepladder that had bent metal spreaders and did not securely lock the ladder in an open position.  b) On the second floor of the coating area - On or about September 24, 2025, the employer exposed employees to a fall hazard, in that employees used a stepladder that had bent metal spreaders and did not securely lock the ladder in an open position.

(a) Throughout the facility - On or about September 24, 2025, the employer exposed employees to fire hazards, in that each fire extinguisher was not mounted with its location identified so it was readily accessible to employees in an emergency.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).  (a) Second floor of the coating area - On or about September 24, 2025, the employer exposed employees to electrical shock hazards, in that all electrical control panels were not closed and secured to guard against accidental contact with live parts inside.
