29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  A.)	Premier Hydraulics LLC, on or about 9/25/25 and times prior thereto: Employees did not lockout prior to conducting servicing operations on equipment including but not limited to CNC Mills and Lathes. Employees are periodically required to conduct cleaning operations from the front and side of the machine.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement

29 CFR  1910.147(c)(7)(i):The training program of the employer did not include all the training elements listed under Items A-C of 29 CFR 1910.147(c)(7)(i):  A.) Premier Hydraulics LLC, on or about 9/25/25 and times prior thereto: The lockout/tagout training program did not cover the definition of servicing and maintenance noted in the 29 CFR 1910.147(b).  As a result, employees were not provided with the knowledge when lockout needs to occur on the machines and equipment present in the workplace.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement

Premier Hydraulics LLC, on or about 9/25/25 and times prior thereto:  Employees were performing service and maintenance activities on equipment including, but not limited to, Hardinge Lather Model SV-200, Fadal Mill Model 904-1, and Hardinger Lathe Model GS-51.  The employer did not certify that periodic inspections of energy control procedures were conducted on an annual basis.     ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
