On or about September 23, 2025, on the roof of a residential building, employees were not in use of fall protection while performing roofing work activities from a work level that was more than six(6) feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about September 23, 2025, at the front of a residential building, employees accessed the roof using extension ladders and the siderails did not extend at least three(3) feet above the landing.
