29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access: (a) On or about September 26, 2025, employees doing cornice work of a new two-stories single family house, while working from a Werner pump ladder jack scaffold system and did not have a ladder for safe means of access.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: (a) On or about September 26, 2025, employees doing cornice work on a single family two stories house from a Werner pump Jack scaffold system and not have personal fall arrest system, guardrail system or safety net.

29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories were not fabricated from metal plates and angles. Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage: (a) On or about September 26, 2025, employees doing cornice work from a pump jack scaffold system where the gripping attachment was made of wood.
