29 CFR 1926.502(d): The employer used body belts as part of a personal fall arrest system. On or about September 25, 2025, and at times prior thereto, on the south side of the roof of residential building number 720, employees were in use of body belts as part of the fall arrest system while performing roofing work activities at heights greater than six (6) feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about September 25, 2025, and at times prior thereto, on the south side of the roof of residential building number 720, employees performing roofing work activities were using portable extension ladders that were not extended at least three (3) feet above the landing.
