OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards: a. 6 Lloyd Road, Montclair, NJ The employer did not ensure that the employee working around a Komatsu PC13US-11 with serial number CT24547153 was protected from struck-by hazards. The operator of the excavator swung the cab while reversing, and the blade of the excavator struck the employee who was walking around the excavator's blind spot. Violation occurred on or about 09-26-2025
