29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   On or about August 20, 2025, on the site of a high-rise residential structure located at 10295 Collins Avenue in Bal Harbour, Florida, employees were exposed to an electrocution hazard when performing paint work from a swing stage scaffold due to the scaffold not being inspected for visible defects by a competent person before the work shift.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   On or about August 20, 2025, on the site of a high-rise residential structure located at 10295 Collins Avenue in Bal Harbour, Florida, employees were exposed to an electrocution hazard when performing paint work from a swing stage scaffold prior to having those employees trained by a qualified person to recognize and avoid the hazards associated with the type of scaffolding being used.
