29 CFR  1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  a) Rialto Manufacturing, Inc. from on or about August 1, 2025 to September 30, 2025, at the address of 1632 Cascade Dr. Marion, OH: The employer did not ensure that operating employees tasked with maintenance and cleaning activities inside the controlled automated zone of the "10k Laser" cell were adequately trained in the application of lockout procedures in which effectively isolate and control hazardous energy sources. Employees not effectively trained on energy isolation application and procedures were exposed to unexpected energization of cell components.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Rialto Manufacturing, Inc. from on or about August 1, 2025 to September 30, 2025, at the address of 1632 Cascade Dr. Marion, OH: The employer did not ensure that energy isolating procedures were implemented prior to employees performing service and maintenance activities inside the controlled automated zone of the "10k Laser" cell. Without the benefit of effective energy isolation, operating employees tasked with bi-weekly cleaning inside the cell were exposed to crushing hazards at or near the material handling trays. The employer's established energy control procedure [required under 29 CFR 1910.147(d)] was not adequately utilized before employees began the aforementioned work inside the demarcated danger zone.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  Rialto Manufacturing, Inc. on or about September 30, 2025, at the address of 1632 Cascade Dr. Marion, OH: The employer did not ensure pinch points created between the automated loading unit and supporting frame on the "10k Laser Cutter" [model- 10089873] were adequately guarded during production operations.  Employees accessing the machine's automation area for in-operational adjustments to production material in this condition were exposed to crushing hazards.
