29 CFR 1910.178(q)(1):Any power-operated industrial truck not in safe operating condition were not removed from service. All repairs shall be made by authorized personnel. At a worksite located at 11901 Grandview Avenue, Grandview, Missouri. Employees were exposed to struck by hazards. Employees operated powered industrial trucks trucks that were not in safe operating condition and were not removed from service as required, such as but not limited to the Toyota Brand fork truck known to the company as #222. Serial #23579, Model # 8FBE15U, which was missing a brake pad.
