On or about July 10, 2025, at 749 Shinnery Oak Way in Waxahachie, Texas, employees installing decking material were observed wearing fall protection but were not tied off, as they worked from heights of more than 6 feet above the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about July 10, 2025 at 749 Shinnery Oak Way in Waxahachie, Texas, the employer did not provide a training program with hazard recognition and avoidance training where employees were performing decking activities 6-feet above the lower level without fall protection.
