One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Associated Production Services, Inc. 100 Louise Drive, Suite 300, IVYLAND, PA 18974: On or about October 2, 2025, a temporary agency employee was exposed to a caught-in/between hazard while cleaning the bucket ladder on Line 5 when the machine activated and the employee was exposed to entanglement within the machine. No method of machine guarding was provided to protect the employee from hazards created by the point of operation or rotating parts.

29 CFR 1910.147(c)(7)(i)(C): An employee whose work operations are or may be in an area where energy control procedures may be utilized, was not instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out. Associated Production Services, Inc. 100 Louise Drive, Suite 300, IVYLAND, PA 18974: On or about October 2, 2025, a temporary agency employee whose work operation was in the area where energy control procedures may be utilized was not instructed about the procedure or about the prohibition relating to attempts to restart or reenergize the machine or equipment which is locked out or tagged out. The employee was not trained on lock-out-tag-out procedures even though they were working on or around the machine.
